Ever since they arrived at Katniss Slums, Macken's drink was spiked with some cyanide. The killer knew he loved Buck's Fizz, so s/he spiked that drink only. The poison was modified to go off at the time it did, and the lights were scheduled to go off by the killer pre-visit. They pulled out a knife, and stabbed him through the chest with a sword held by a knight in armour standing at the door.
