With 2 HOHs, they will come up with their own nominations, meaning there will be 4 people on the block.
After that, they compete in the Battle of the Block.
If an HOH wins, the other HOH's nominations are void AND the winner can then use that power to change their own nominations (Much like POV).
But if a nominee wins, then they are removed from the block (and that HOH replaces them) and the winner decides which HOH's nomination set is the final one. The HOH on the losing side is then eligible for nominations should a veto be used.
