Keys appear 2-4 times in a single tengaged day. If you're active, you'll realize it. Every update (health/apple will go up, number of checks are updated), a key or apple will appear surrounded by bottle of poison (don't press those bottles, or your health which is your apple will deduct), or sometimes it will just update but no key/apple will be shown. Based on my own observation, updates can happen every 30 minutes to an hour as an interval.
And to update your skin, just go to your settings, go to change image, change your skin, then save. :)
