How did Becky's head get hit by a train

First off, wouldnt you not see the train coming from a far?
Secondly how does your head even get hit by a train?

It was a street car kind of thing, they're designed to be quiet so they dont wake up residents & it was snowing out so she couldn't hear or see it :P
